Who is eligible for services at Well Living Group Home?
Well Living Group Home serves adults 18 years of age and older with intellectual and developmental disabilities. Individuals may receive services through the Florida Agency for Persons with Disabilities (APD) or through private pay arrangements, depending on eligibility and funding source.

Are you licensed by the Agency for Persons with Disabilities (APD)?
Yes. Well Living Group Home is an authorized residential service provider with the Florida Agency for Persons with Disabilities (APD). All services are delivered in compliance with APD rules, regulations, and quality assurance standards.

 

What types of funding do you accept?
We accept:

  • APD Waiver-funded services (with approved authorization)

  • Private pay for individuals age 18 and over

Funding options are discussed during the intake process to ensure transparency and appropriate service alignment.

How does the intake and placement process work?
The intake process includes:

  1. Initial inquiry and consultation

  2. Review of APD documentation or private pay needs

  3. Assessment of individual support needs and compatibility

  4. Coordination with support coordinators and families

  5. Admission and development of an individualized support plan

All placements are based on safety, appropriateness, and the ability to meet the individual’s needs.

 

Do you provide person-centered support plans?
Yes. All clients receive person-centered, individualized support plans that align with APD service authorizations and personal goals. Plans are reviewed regularly and updated as needed in collaboration with support coordinators and families.

What level of supervision is provided?
Supervision levels are determined by individual needs and APD authorizations. Our staff provide appropriate oversight to ensure safety while promoting independence and personal choice.

Are staff trained and qualified?
Yes. All staff receive required training in accordance with APD regulations, including:

How do you communicate with families and support coordinators?

What services are not provided?
While we offer comprehensive residential and supportive services, we do not provide:

  • Medical or nursing services beyond permitted support

  • Skilled clinical or therapeutic services unless coordinated through external providers

We collaborate with licensed professionals as needed to support the individual’s overall care.

How do private pay services work?
Private pay services are available for individuals age 18 and older who are not receiving APD funding or who wish to supplement authorized services. Rates, services, and payment terms are discussed upfront and outlined in a private pay agreement.
